Welcome Back from the School Nurse

Hello Wildcat Parents,

I hope this email finds you healthy and excited as we enter a new phase in this extraordinary school year! As we prepare for some of you to come back into the building, I want to remind you that the rules for medication taken during school hours haven't changed. All medication must be kept in the nurse's office unless your child has a "
permission to carry form" on file with us. I am attaching this form as well as the medication administration form to this email. The "permission to carry" form doesn't expire; therefore, we can use the same form as long as the student is in a DeKalb county school. You can bring me a copy if this is already completed. If the student is returning, I should have a copy. The medication administration form expires and must be completed every calendar year. I need a separate medication form completed and signed by the doctor for every medication the student keeps at school, including over-the-counter medication. I need a "permission to carry" form for each medication the student keeps with them during the day. You may bring the medication with the corresponding forms to the office, and the office staff will place in my box. All medicine needs to be in the original labeled bottle and cannot have expired. If the student carries an inhaler and/or an EpiPen on them, I would if possible, bring in a back-up to keep in the clinic.

If your student is diabetic, please send supplies such as a glucometer, extra insulin, snacks, juice, pump supplies, ketone strips and glucagon. If you wish for me to follow their blood sugar during the day, I can do that as well.
